Photo and Media Release Permission to take and use photographs and recordings I, ____ (Your full name), give ____ (Organisation or photographer) (the “Organisation”) permission to take photographs, video and sound recordings of me (the “Media”) at ____ (Event or occasion) on ____ (Date), and to use them as set out in this release. Section 1: Where the Media may be used 1.1 The Organisation may use the Media only in the places I have ticked below. It will not edit the Media in a way that changes what I did or said, or that makes me look foolish, or add anything that suggests I said or endorse something I did not. Section 2: Whether my name is used 2.1 The Organisation may use my name or other details that identify me in a caption only if I have ticked here: ____ (My name may be used in captions) My name may be used in captions. Otherwise it will use the Media without my name. Section 3: Respect and safety 3.1 The Organisation will not photograph or record me in a place where I would reasonably expect privacy, such as a changing area or a toilet, and will stop taking pictures of me at any time if I ask it to on the day. 3.2 The Organisation will choose pictures that show me in a fair and positive way. It will not use a picture that I have told it, in writing, that I find embarrassing or that shows me in a situation I did not know was being recorded. 3.3 If the Organisation is not the only person taking pictures, for example at a public event, it is responsible only for the pictures that it or its own photographers take. Section 4: How the pictures are stored and who sees them 4.1 The Organisation will store the Media securely, keep it only for as long as it needs it for the uses I have ticked, and then delete it or archive it where it cannot be seen by the public. 4.2 Only the Organisation’s own staff, volunteers and contractors who need the Media for those uses will have access, and they will be bound to keep it private except for the uses I have allowed. 4.3 Pictures that are put on the internet can be seen by anyone, copied and shared, and the Organisation cannot control what other people do with them once they are public. I have thought about this in making my choices. Section 5: What I understand 5.1 I will not be paid for this permission and I do not own the Media, which belongs to the Organisation. I have no right to approve each use, but I can ask to see how I have been shown. 5.2 This permission lasts ____ (How long this lasts). 5.3 I may withdraw this permission at any time by writing to the Organisation at the email address below. The Organisation will stop new uses, and will take the Media down from the places it controls within thirty days. It cannot recall printed material already distributed, or remove copies others have made. 5.4 I am at least eighteen years old and I am entitled to give this permission. If I am under eighteen, a parent or guardian must sign the separate release for minors.
